How to Foam Roll the TFL and IT Band


The Tensor Fascia Latae is a small muscle on the lateral side of the hip, which forms the Iliotibial band, running down and inserting on the lateral portion of the tibia. The TFL and IT band are very often tight and overactive, which can lead to limited range of motion in the legs and the inability to squat and get in other positions with the knees bent. Knee pain is also associated with overactivity in this area, which is referred to as IT band syndrome in some cases. Foam rolling this are can reduce neurological activity and reduce many movement compensations associated with its tightness.

To foam roll the TFL:

Lie on the floor on your side. Place the foam roller under the hip, with the opposite foot placed on the floor in front of the leg being rolled in a bent position.

Support yourself on your forearm and foot, and place your weight on the foam roller slowly. The foam roller should be positioned in about the same place your hand would be if it were in your pocket.

As you put your weight on the foam roller, roll down toward the knee slowly and find the point of greatest tenderness. This is the trigger point.

After locating the trigger point, relax the muscles of the leg and hip, and put as much pressure as possible on this point, holding it for 20-30 seconds. Use your opposite leg to press down and relieve pressure from your hip if the pain becomes too extreme. Remember to keep the muscle relaxed the entire time.

Switch legs and repeat!

To foam roll the IT band:

The procedure is the same as that for the TFL, but you will roll down further toward the knee. Often, you will find a trigger point in this area a few inches above the knee.

Remember to relax the muscles of the leg and hold the pressure on the trigger point for 20-30 seconds.

Switch legs and repeat.

The TFL and IT band are very commonly overactive, and will often cause the knees to move medially or laterally during a squat. In addition, and tight TFL or IT band can pull on the knee joint and cause knee pain, especially on the lateral side of the knee. Foam rolling this area daily can often resolve these problems in a relatively short period of time, and is worth the effort (and pain). As always, a full assessment should be performed by a qualified professional in order to get the best results.

Runners knee or Illiotibial Band Syndrome takes its toll on many runners. Unfortunately, the lack of information on how to deal with this problem before it happens causes it to last far longer than it should, or to develop into a reoccurring problem.

What is it?  

Runners knee is a condition in which a sharp, burning pain manifests in the knee or the hip. It occurs when there is irritation to the illiotibial band, or IT Band, a band of fibrous tissue that runs from the hip, along the outside of the shin bone to just below the knee joint. Its function is both to provide stability to the knee and to assist in flexion of the knee joint. When irritated or inflamed, it does not glide smoothly and movement of the knee joint becomes painful. Usually the pain worsens with continued movement.

Over-training is said to be the most common cause, but running on a banked surface, inadequate warm-up or cool-down, or certain physical abnormalities may also contribute.  

As usual we here in America tend to focus on what is to be done after a problem occurs instead of taking a few moments to learn about our sport and take some preventative measures. So how about putting in a little bit of work on the IT band to keep it supple and flexible? Why wait until you have a problem that will take you out of the running loop for up to a month and make you reduce your mileage drastically? I would tend to disagree that overuse is the main cause. This is a half truth. The correct way to state this would be that overuse without the proper foundations is the leading cause of runners knee. You can run 50 miles without training and if you have worked on the IT Band, it will not act up! 

Foam Rolling is one of the best and fastest ways to keep the IT Band healthy, flexible and supple. If you are an avid runner, this should be one of the cornerstones of your routine. Foam Rolling the IT Band helps to keep the knots and kinks worked out and is a tremendous aid in keeping it flexible. If you have had problems with the IT Band in the past, this is a godsend. Also, try to incorporate a good stretching routine into your activities. 

Staying Healthy:

If indeed you do end up with runners knee, you have got to rest it. If you rush back to running, you'll never break the injury cycle. Patience is tough, but is required. When you do start back running, reduce your miles and ice your knee frequently to reduce the inflammation. Work in light stretching and foam rolling when pain does not force you to stop. It is possible to keep running, but cut your run short as soon as you begin to feel any pain. Reduce the amount of hill work you do until recovery is complete. Winter Joint Pain Help!


Before I solved my knee pain and knee clicking disorders I remember the very cold times of year being the Most horrible. Dry skin, stiff legs, aching muscles, BLAH! I thought I would come on right now and write out a handful of simple tips for managing your knee joints in the winter time and cold air to stop soreness and clicking. Sit on the floor with your legs completely straight in a V-formation and just hold that position for as long as you can. You will really feel when you're done with this one. This is another one that can be done anywhere and that you should try and do at least once a day, maybe in the morning to help get yourself moving.

Simple Diet Changes

The inflammation that winter months causes is also (sometimes) very easily solved via simple diet changes and nutrition. Much like you use oil to lube up the pieces in a car you can add olive oil into the foods you eat to do the same on your knee joints. Joint Pains Treatment - Home and Ayurvedic Remedies


Pains in the joints are very common complaints as a person advances in years. More often than not, these joint pains are due to arthritis. There are several kinds of arthritis which may occur in old age. In all these types, the indication is that the joints have become worn down with age and so they are not attacking themselves. The Ayurvedic name for arthritic joint pains is Sandhi Vata. As the name suggests, it is caused by vitiation of the vata dosha in the human body. The word sandhi means 'joint' in Sanskrit. Ayurveda believes that Sandhi Vata can be caused due to rheumatism, infections, gout, hemophilia, trauma and even due to digestive problems which result in a buildup of ama in the body.

(1) Useful Herbs in the Treatment of Joint Pains

1. Alfalfa (Medicago satina)

The alfalfa herb is very effective in the reduction of joint pains. It must be had in the form of a tea four times a day.

2. Ashwagandha (Withania somnifera)

Ashwagandha is the herb popularly known as Winter Cherry in the west. It has several positive properties for the human system. Treating joint pains are included in that list of properties.

3. Banyan (Ficus bengalensis)

The banyan tree produces a sap much like the latex from rubber. This is used as an external application over the joints. This application relieves the pains in the joints after a few regular applications and massages.

4. Bishop's Weed (Trachyspermum ammi)

Oil extracted from the bishop's weed is applied locally on the affected joints to relieve pains in them.

5. Celery (Apium graveolens)

Celery can be used as an effective treatment for rheumatism and gout, both of which can cause pain in the joints. Thus, celery provides more lasting treatment from the problem. The alkaline content of celery makes it effective in treating joint pains.

6. Dandelion (Taraxacum officinale)

Dandelion is rich in magnesium, an element that is needed for the proper mineralization of the bones. Use of dandelion makes the bones stronger and protects them from pains.

7. Garlic (Allium sativum)

Garlic is a very effective remedy for the treatment of joint pains. About five to six cloves of garlic must be eaten every morning to seek relief.

8. Ginger (Zingiber officinale)

Ginger is also one of the best methods in the treatment of joint pains. It must be included freely in the daily diet of the person. Tea made with ginger in it is also beneficial.

(2) Dietary Treatments for Joint Pains

Ginger and garlic are extremely good for controlling joint pains. Some people find the odor of ginger intolerable. In order to reduce the odor, the cloves of garlic could be slightly fried in butter. All bitter tastes are good for arthritis. Hence, people could consume bitter gourd and bitter drumsticks without any worry.

People with joint pains should totally steer away from all kinds of sour foods, since these can increase the vata. Constipation can aggravate the pains. Hence foods that are difficult to digest must be avoided. This also extends to protein-rich foods such as pulses. Foods must not be fried so as not to vitiate the vata further.

(3) Ayurvedic Treatments for Joint Pains

Ayurveda prescribed different kinds of treatments for different kinds of joint pains. When a patient approaches an Ayurvedic doctor, the first thing to do is to ascertain the cause of the pain. Depending on that, the medicines are prescribed.

1. Rheumatic arthritis is known as Ama Vata in Ayurveda. It is caused due to a low digestive fire, due to which ama is retained in the body. For its treatment, first the ama is removed by fasting and purgation if needed. Then a mixture of punarnava, guggulu, galangala and garlic is administered in dosages of one gram per day. Dashamoolarishta is also prescribed in 15 milliliters quantity thrice a day for three weeks.

2. If the joint pain is due to trauma, then Vishagarbha taila is prescribed for external application over the region where the joint pain is felt. For internal administration, the patient is prescribed Rasnadi Guggulu and Guduchi tablets, which are to be taken in a quantity of 250 milligrams thrice or four times a day with water.

3. In people with age above fifty years, a type of arthritic joint pains known as osteoarthritis is observed. This is known as Ashtigata Vata in Sanskrit. For this condition, either Pravala Panchamrita or Lakshadi Guggulu is prescribed. The dosages are 125 milligrams twice a day for one month and 250 milligrams thrice a day for three months respectively.

(4) Home Remedies for Joint Pains

a. Massage the joints with a mixture of camphor, eucalyptus oil and menthol before going to bed.

b. Have a glass of carrot juice early in the morning. Continue this till you find reduction in the pains, which should happen within a week.

c. Do not exercise wildly, but at the same time do not keep your joints lethargic. Perform some activity with them so as to keep them mobile.

d. Have a bath with warm water every morning. This facilitates proper blood circulation.

The hip joint plays a major role in weight-bearing and walking.

It's a ball-and-socket joint that combines great stability and a wide range of motion. This joint is comprised of the head of the thigh bone (femur) which is shaped like a ball. This sits inside a cup-like area of the pelvis called the acetabulum ("socket"). It is surrounded by powerful muscles that help maintain the body in an upright posture and also help with walking. There are many muscle groups and ligaments that help control the movement of the patella. The patella and the femur come together to form a joint known as the patellofemoral joint. This joint is made up of muscles, soft tissue attachments and the trochlear groove where the patella moves along. PFPS is a knee condition in which people usually indicate that they have pain "around" or "under" the kneecap. Sometimes this condition is also referred to as chondromalcia patella, but various medical authors have disagreed on the validity of this statement...

Typically, your kneecap is designed to move smoothly along a groove on the femur or thighbone, called the trochlear groove. PFPS, is believed to be related to poor kneecap (patella) "tracking".

This can occur when the front thigh muscles are weak and imbalanced, causing pressure and strain on the cartilage on the undersurface of the kneecap. - Patellar dislocations are more likely to occur when your knee is moving; this is more commonly seen in runners and athletes.

Causes of Poor Knee Tracking

There are four conditions that can predispose poor tracking of the patella along the femur, and it is important to see your physician if you believe you have one of these problems.

- Weak Quadriceps- The front thigh muscle or the quadriceps become weak and can cause an imbalance as the patella slides over the groove along the femur. A supervised strengthening exercise can help correct this poor knee tracking issue.

- Imbalanced Quadriceps - The four divisions of the quadriceps can become imbalanced. The kneecap is pulled off center as a result of the imbalanced pull by the outer quad and the inner quad. This condition deteriorates over time to become full blown PFPS.

Rehabilitative exercise can also help correct this imbalance of the quadriceps and correct the painful knee experience. If this is done at the early stages of the muscle problem you may be having, the condition may not deteriorate into PFPS.

- Weak Hamstrings - This imbalance occurs when the muscles in the front of the thigh (quadriceps) becomes stronger than the hamstrings. This condition can put a strain on the quads causing increased pressure on the femur and the kneecap.

Treatment of PFPS

There are two types of medical interventions used to help treat Patellofemoral pain syndrome- invasive and non invasive treatments.

Under a more severe condition, the surgical option might be recommended by your orthopedist (consult your physician for medical advice). The surgeon can either perform an arthroscopic lateral release, which will help correct the tracking of the patella, or do ligament tightening.

A support, sometimes referred to as a patellar tracking knee sleeve (or patella tracking knee brace) can be used to help limit any deviation of your kneecap. Stretches are very important because tight muscles are easily injured. They can also pull a joint out of alignment. Both these factors would further increase your pain, if you are already symptomatic. Stretching over two joints at a time is more effective, as is working with two muscle groups at the same time. Always stretch the uninvolved leg as well. To be effective, each stretch is held for 30 seconds with 5 repetitions.

The main muscles that effect knee stability are:

1. The quadriceps and hamstrings. These are muscles in the front and back of your thigh respectively.
2. The hip flexors made up mainly of the iliopsoas and rectus femoris. These are located in front of your hip
3. The hip extensors (the buttocks) mainly comprise the gluteus maximus and the hamstrings
4. The hip abductors which include the gluteus medius and IT band (by virtue of the muscles attaching to it). These are on the outer side of your hip.
5. The hip adductors that are located on the inner side of your thigh, beginning in the groin
6. The calf muscles

Stretches for knee pain are carried out slowly.The exercises below are for pain in the right knee, no matter what the cause is.

Stretching the hip flexors and quadriceps together, stretches out the muscles in front of the hip and thigh:

1. Start in half kneeling position, i.e. bear weight on the right leg, by kneeling on the right(R) knee, while you support yourself on the left(L) leg by bending it at 90 degrees at the hip and knee. (Place a cushion under your (R) knee, for comfort, if necessary).
2. The foot of the (R) leg is placed up on a chair behind you, while you continue to stay in half-kneeling.
3. Arch your back backward gently, to stretch out the(R) hip flexors and (R) quadriceps. You can reach back to hold the chair if you wish. Hold/ repeat as suggested above
4. Next, placing both hands over your (L) knee, lean forward over (L) leg, so that your (L) hip is well flexed beyond the position of the ankle of the same leg. The (R) knee does not move out of position. The (R) foot continues to stay up on the chair behind you. This will stretch the (R) hip flexors. Hold/repeat as is required.

Stretching the hamstrings and calf muscles together, stretches out the back of the knee:

1. Place the toes of your right foot on a roll that is 4-6" in diameter while standing on your (R) heel. Place your (L) leg comfortably behind you.
2. Tighten your (R) quadriceps to relax your(R) hamstrings(by the muscle law of reciprocal relaxation)
3. Gently drop down towards the floor by bending forward at both hips (without bending your knees) till your fingers reach the floor. Hold/repeat as suggested above.

Along with stretches for knee pain, strengthening of all the muscles around the hip, knee and ankle is also very important. Strength is built up gradually.But for the first and last two exercises, all the rest (exercises 2-6) are started at 5 counts and increased by 1 every other day till 15-20 counts are reached. Weights can slowly be introduced as a progression. Again we are discussing the right knee.

1. Lie on your back, after placing a small towel roll under your (R) knee. Gently press your knee down toward the surface using your quadriceps, while lifting the heel of the same leg off the surface. Hold the position for 10 seconds. Move your foot back and forth during those 10 seconds. Repeat both steps 10 times. This is one set. Perform 5 sets. These are known as "knee isometrics".
2. Still lying on your back with your legs out straight, bend your (R) leg up toward your chest. Return back to starting position. Repeat. Stay mindful of the counts.
3. Next, lift your (R) leg up towards the ceiling without bending your knee. Stop when your leg is at a 45 degree angle with the surface you are lying on. Return to starting point. Repeat. Count as above.
4. Turn on to your left side. Bend your left knee to provide support. Lift your (R) leg sideways, while keeping your knee straight. Return to base. Repeat. Stay with your counts.
5. Now lie on your stomach. Bend your (R) knee, then lift that leg upward off the surface. Return. Repeat. Count.
6. Continuing to lie on your stomach, lift your (R) leg off the surface, this time with your knee straight. Return back and repeat. Keep your counts.
7. Wall Slides: Stand against a wall. Place a 9" diameter ball in the small of your back. Bend your knees 30 degrees. Slide down, then return back up the wall. Place your hands on the wall for support. Keep your feet parallel. Never let your knees go out over your toes. Repeat 10 times.
8. Stand on your toes while holding on to the back of a chair for support. Bring your heels back down. Repeat 10 times.This strengthens your calf muscles.

This completes your hip, knee and ankle strengthening program.

Pain is your body's signal for help. Doing a regular exercise routine will ensure the health and stability of your knee joints and decrease or prevent pain as the case may be. Since strengthening shortens muscles, it is advisable to start with "knee isometrics" to warm up, continue with the strengthening routine and finish with the stretches for knee pain.
